Equipment typically arrives within 1–2 weeks of the order, assuming no documentation or insurance delays. We keep you informed throughout.

What we need to process your order

To process a referral efficiently, we require three documents. All forms must be signed by the treating clinician.

Prescription / Order Form

Available through e-prescribing platforms (Parachute Health recommended, DMEscripts), EMR systems, or traditional fax. Must be signed by the treating clinician and include the specific equipment or supply category ordered.

Patient Face Sheet

Includes patient demographics, contact information, and health insurance details. This allows us to verify coverage and contact the patient promptly after receiving the order.

Patient Chart Notes

Documentation supporting medical necessity for equipment or supplies coverage, dated within six months prior to the order date. Clinical documentation requirements vary by supply type and insurance plan.
